          I wish to request all the members of Indian Academy of Neurology to send me the completed performa with two passport size photographs by 30th of June 2004. So far the response has not been good and if I do not receive completed Performa and photographs, your name may be left out from the directory. The directory Performa has again enclosed as a loose sheet in the news bulletin of April 2004.
